The individual was found wandering in Medical Plaza 4 near Memorial Hermann Southwest Hospital in Houston on September 8, 2017 around 1:00 PM. He later died in a Waller personal care home on October 9, 2017.

Height: 5’9”
Age: Estimated 50-70 years
Race: Hispanic
ADDITIONAL IDENTIFIERS:
The decedent has wavy brown, graying hair, a mustache and beard. He has a large mole on the bridge of his nose (right side) and a smaller mole under his right eye. He has a healed broken nose. He spoke Spanish and reported three different names: Jose Rodriguez, Jose Perez, and Maximo Dominguez Perez. He may have relatives in McAllen, Texas.
CASE DETAILS:
When the individual was found wandering with altered mental status on September 8, he was wearing a hospital gown, an adult diaper, and non-slip socks. He was admitted to the hospital and then discharged to a personal care home in Waller on September 20, 2017 where he declined and died on October 9, 2017.
